(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-i 9ed-peg <br />INSPECTION DATE: ~2-~-a~ INSPECTORS INITIALS: ~- <br />OBSERVATIONS <br />Completed a partial Inspection of this operation to observe progress of reclamation activities. <br />Met with Mr. Blair Cooley to discuss the construction of the jurisdictional dam on the north side of the reclaimed <br />gravel pit. Discussed the proposed plans, specifications and testing frequencies of the State Engineer's Office. <br />The crest of the dam is approximately the same elevation as the approved embankment structure. <br />Discussed the slurry wall (location, etc.) and progress of final reclamation. <br />The approved post-mining land use far the site is water storage. The facility has been tested and approved for <br />water storage by the SEO. Informed the operator that the financial warranty con be reduced now that the slurry <br />wall has been totally installed and the facility has been tested and approved by the SEO. A bond reduction <br />request may be submitted to the Division at any time.
