(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1487-775 <br />INSPECTION DATE: 09/75/01 INSPECTOR'S INITIALS: KAP <br />OBSERVATIONS <br />This inspection of the Stout-Amick Pit was in response to the owner's request to reactivate the permitting process <br />which was never completed at the time of the original application. The original application was approved but bond <br />was never posted, and therefore a permit was never issued. Present at the inspection were Kate Pickford, DMG, Greg <br />Squire, DMG, and Diana Watkins and Jeff Watkins, owner/operators. Weather at the time of inspection was clear and <br />about 80 degrees. <br />2. There was no evidence of mining activities ever having occurred within the proposed permit boundaries. There was <br />evidence of some minimal mining activity along the access road to the site, south of tVie proposed permit area, which <br />appears to be a result of old activity. Access onto the permit area was via a roadway entering from the south. The <br />owner indicated that upon receipt of the permit and commencement of mining, a new access road would be installed <br />entering from the east, a stipulation indicated on the Rio Blanco County approval of the original permit. The only <br />man-made structure within 200 feet of the permit area is a fence which is owned by the property owner and potential <br />operator of the proposed pit. <br />3. The current owner/operator is not the same individual who applied for the original/approved permit. The current <br />owner was advised that she would need to file a Succession of Permit Application with the Division of Minerals and <br />Geology. The applicant will also submit a new Permit Application Form. The applicant will soon receive an <br />adequacy review letter indicating deficiencies in the current file for the site.
