:. t. <br />~` <br />(Page 2) <br />NINE ID ~ OR PROSPECTING ID f M-80-244 <br />INSPECTION DATE 10-21-94 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />A monitoring inspection of the Phase 1 leach pad construction and appurtenant installations <br />was conducted. The focus of this inspection was clean out of the stormwater sump above the <br />toe berm (hereafter referred to as the "sump^ for purposes of this report) in preparation for <br />composite liner installation. <br />Upon arrival at the site it was observed that a backhoe had been used to clean the bulk of <br />the saturated silts and clays from the sump. The material that had been dredged was trucked <br />from the leach pad and dumped below the toe berm in the area of the Carlton tails. A smooth <br />edged fitting had been bolted to the bucket to cover the teeth and segments in an effort to <br />minimize damage to the VLDPE liner at the bottom of the sump. A materials cross-section in <br />the sump from top to bottom ie: 1) saturated fine grained soils, 2) approximately two feet <br />of compacted soil liner fill, 3) geomembrane, 4) geotextile, 5) 1 foot of select structural <br />fill, 6) underdrain fill. A perforated riser pipe had been installed through the entire <br />cross-section and had been fitted on to the horizontal 24" perforated underdrain pipe. The <br />critical aspects of the sump clean out are that saturated or otherwise unacceptable fill ie <br />removed and replaced with the appropriate material, and that any damaged geotextile or <br />geomembrane ie adequately repaired or replaced. <br />The bolt on edge on the backhoe had fallen off at some point, and the backhoe had ripped the <br />geomembrane in at least two locations near the riser pipe. Once the backhoe had gone ae deep <br />ae it could without further damaging the VLDPE, a bob-cat and hand shoveling were employed <br />to clean down to the underdrain VLDPE cover. Once a 15' x 15' area was cleaned off, the <br />geomembrane and geotextile were cut away to exposed the select structural fill. There was <br />a small amount of mud on the select fill surface in the immediate vicinity of the riser pipe, <br />and a veneer of mud on the select fill where the VLDPE had been ripped by the backhoe, <br />otherwise, the surface of the select structural fill was markedly dry. A small backhoe was <br />brought in the dig out around the riser pipe. This stage in the project was reached at <br />approximately 2:30 p.m. I left the site at this point, and Jim Pendleton arrived to observe <br />the removal of the riser pipe at approximately 3:15 p.m. Dr. Pendleton's observations will <br />be the subject of a separate report. <br />Also inspected on this date was the temporary topsoil stockpile below the primary crusher <br />pad. The catchment road and berme above the topsoil pile had been adequately maintained to <br />prevent contamination of the topsoil from sloughing off the crusher pad fill. The contractor <br />was stripping topsoil with a small dozer in advance of future fill placement to the east. <br />A water sample was obtained from the outfall of the 24" perforated underdrain pipe. The <br />sample was collected at 14:20, and was preserved to pH 12 with NaOH to facilitate analysis <br />for total and W.A.D. cyanide. The sample was put on ice at approximately 14:40 in Cripple <br />Creek, and was maintained on ice or in a secure refrigerator until it was delivered to Core <br />Labe at 15:10 on 10-25-94.
