<br />,' (Page 2) • <br />MINE ID !1 OR PROSPECTING ID # M-Y 981-021 <br />INSPECTION DATE 12/19/01 <br />INSPECTOR'S INITIALS RCO <br />This partial inspection was performed by the Division as part of its monitoring of Hard Rock and Metal 112 permits. The <br />operator was contacted but was not present during the inspection. The site was not active at the time of the inspection. <br />Only the portal area was inspected, and not the upper roads or vent shaft sites (made part of the permitted area in the <br />original application, and technical revisions nos. 1 and 21. <br />The previous operator's ID sign was posted on the locked gate to the portal area, but no ID sign for the current permittee. <br />No permit boundary markers were noted, either in the boundary locations shown on 1981 and 1983 maps or the different <br />boundary shown on the 1998 map. Due to the lack of a current and complete permit ID sign and no visible corner markers, <br />the topic of "signs and markers" is noted as problem in this report. Please see the last page for the correction date. <br />In an attempt to update the permit maps, the operator provided a new map in 1998. The operator was informed of the <br />numerous inadequacies on the 1998 map (especially the amended boundaries at the portal areal) but the maps were never <br />corrected. The operator has noted thax there are no changes, and therefore no map to provide, in annual reports filed <br />subsequent to the review of the 1998 map. Due to the inaccurate and incomplete map in the file, this report notes a <br />problem under "records." Please see tfie last pace for the correction date. (It is important that complete and accurate <br />maps be prepared, due to the possibility that the eastern toe of the waste dump might extend beyond the approved permit <br />boundary, and that total acreage might be in excess of the approved 20-acre affected areal <br />The pad area contains a metal shop building, fuel and oil storage, parts storage, etc. The fenced transformer platform <br />contains several transformers, two of which appear leaky. They are tagged, not with yellow indicating PBC, not with blue <br />indicating PCB-free, but with a black "te:sted" tag. Since leaks and eventual removal during reclamation are affected by <br />whether PCBs are present, the operator roust provide information as to the type of transformers these are. (This is an item <br />that is required to be addressed, and is included under the "records" problem in the previous paragraph.) <br />The north end of the shop building is the waste oil storage area, except that the time to stop placing more waste oil in the <br />55-gallon barrel ended awhile ago. There is a barrel full to overflowing, to the point that oil has flowed across the ground. <br />The surrounding area is stained indicating that this practice has occurred over time. There is no spill containment present, <br />and no apparent evidence at clean-up. Ttris is Door hydrocarbon management, and is noted as a problem in this report under <br />acid or toxic materials. All of the contaminated soil must be properly disposed of. All waste oil must be properly disposed <br />of. Please see the last page for the correction date. <br />The portal is not secured against unautf~orized entry: the lock is not connected to anything and the gates swing open. <br />The operator should secure this opening since it presents a potential hazard. This is not noted as a formal problem in this <br />report, but is noted simply as an item which should be addressed, soon. <br />The bond will be reviewed soon, and the operator will be informed of any increase, at which time the operator must provide <br />the additional bond within 60 days. <br />There were no additional items observed.
