(Page 2) <br />MINE ID # OR PROSPECTING ID #: Ad-tQV~-Sd~ <br />INSPECTION DATE: t/n/n~ INSPECTORS INITIALS: ~Ap <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Minerals and Geology's four-year monitoring <br />program. Present at the inspection were Kate Pickford, DMG, and Gayle Lyman, Elam Construction, Inc <br />(operator representative for adjacent 1 1 1 permit). Weather at the time of the inspection was clear, <br />calm, and approximately 40 degrees. <br />2. The site is surrounded by a 111 permit (M-2005-029), operated by Elam Construction. Elam Construction <br />has completed the project for which the 1 1 1 permit was obtained, and is considering whether or not to <br />continue mining at the site by obtaining a 112c permit which encompasses this 1 l Oc permit, the active <br />1 1 1 permit, and some additional, adjacent acres. <br />3. During the activities related to the 111, Elam Construction permit, the remaining materials in this permit <br />were mined out, and the resulting pit was filled in by Elam. Some of the materials excavated from this <br />permit are stockpiled near the site of the previous excavation. The remainder of the materials <br />stockpiled in the area were obtained from the 1 1 1 permit. <br />4. The site encompassed by this permit has been fully mined, backfilled, and graded to a flat surface. The <br />permit was revised in 1995, specifying that a stock pond would be part of the reclaimed site. If Elam <br />incorporates this permit into a new 1 12c, the issue of reclamation of this area will be addressed under <br />that permit. If, however, the current operator desires to have the site released, the operator may install <br />a stock pond, as indicated in the reclamation plan, or revise the current permit to eliminate the stock <br />pond. Part of the revision to the reclamation plan would require a letter from the landowner agreeing <br />to the change. <br />5. The next step regarding this permit will depend on Elam's decision as to whether or not to continue <br />operations at the site.
