ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_I9Rn_~ed <br />INSPECTION DATE: 10,!28/n5 INSPECTOROS INITIALS: RK <br />OBSERVATION <br />The Division conducted a monitoring inspection of the site while in the area to discuss the upcoming technical <br />Revision. CC&V plans to submit a Technical Revision that will include the affected area configurations for the <br />mining areas and other minor changed that will be included in the TR such as the recent placement of <br />additional pipes to facilitate solution flow in the Phase IVC area, to avoid spills outside the containment area. <br />During the meeting both the Division and CC&V agreed that it would be best in addition to the, MAPS <br />submitted in the annual report, it was also a good idea, for DMG to get on annual basis An updated Facilities <br />mad, that shows the current affected acreage for the year. Given the dynamic process of the operation, and <br />requires constant changes depending on where the ore body to be mined at a given time, which might <br />require construction of a new haul road and some ancillary support structures, yearly updated facilities map <br />would of help to every one In anticipation of such un-for seen changes, CC&V has in place a surplus warranty <br />in the amount of $284,000. Under the MLRD BOARD APPROVED AMNEDMENT #8, OUT OF THE TOTAL PERMIT AREA <br />OF 5847 ACRES, 4184 ACRES CAN BE AFFECTED. <br />The physical inspection of the site began around the Phase IV inter Phase liner ditch area, where the recent spill <br />out side the containment occurred. To make sure the area will not cause to dam up, the operator has added <br />another 150 feet extension of the b inch perforated pipe for a total of approximately 400 feet. The pipe <br />appeared to be functioning as designed. Inspected the main Cresson area, the storm water ditch along the <br />west side was under going a routine maintenance and some alignment the sump for better function. The old <br />head frame that was moved in anticipation of the west Cresson pit expansion was now in its final resting place, <br />pit was active at the time of the inspection. Inspected the East Cresson area. CC&V had planned to back fill <br />the pit to original Contour, which would have been approximately 200 feet about what was approved under <br />Amendment # 8, however, due to the sequence of mining in the area, CC&V now wants to only backfill a very <br />small area only 50 feet above what was approved in Amendment # 8, when the TR, IS SUBMITTED. <br />Inspected the Iron Clad Globe Hill ponds, CC&V has placed BIO RINGS to lower the Nitrogen content in the <br />ponds. Three bio ring islands were noted. Inspected the Phase I,II, III AND IV LEACH PAD AREAS and collected <br />the following readings from the High and Low volume solution Collection Transducers. <br />Phase I High Volume Solution Collection transducer # 1 as noted in the earlier inspection reports, has not been <br />replaced yet, it will be replaced when the pumps are required to be replaced, # 2 was at 52.2 and # 3 was at <br />52.8.Low volume solution collection transducer # 1 was recorded at .68 and # 2 was at .27.Piezometer for the <br />pond was at 52.0 feet. <br />Phase II &III High Volume Solution Collection transducer # 4 was recorded at 38.1 #5 was at 35.3 and # b was at <br />39.3. Low volume transducer # 1 was at .15 and # 2 was at .27. Piezometer for the pond was at 39.2 feet. <br />Phase IV High Volume Solution Collection Transducer # 7 was at 40.4 #8 was at 39.4 #9 was at 40.1 and # 10 <br />was at 40.4. Low volume transducer # 1 was at 13.8 and #2 was at 6.3 inches of hydrostatic head. (All other <br />Piezometer readings are In feet of hydrostatic head unless other wise noted) <br />Inspected the external pond, recently the pond received barren solution, due to operator error, not closing <br />both valves. According to CC&V, the concentration of the barren solution was approximately 12.ppm wad <br />cyanide and by the time the solution was in the pond, the pond wad cyanide concentration was 1.2 ppm. The <br />pond is approved to accept 40-ppm wad cyanide concentration, in consultation with DOW, to protect wild <br />life. However, CC&V don't want any process solution in the pond unless absolutely necessary. After the error <br />was noted, according to CC&V, they added Calcium Hypo chloride to detoxify the cyanide and implemented <br />new procedures when valves are open and closed in the barren solution containmenT tanks. While in the area, <br />collected the following readings from the low volume solution collection transducer for the pond # 1 was at <br />13.2 and # 2 was at 15.7 inches of hydrostatic head. <br />Inspected the under drain area both the south and four inch pipes were discharging, north under drain was <br />dry. All the lower ponds were also dry. <br />
