• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 10/28/98 INSPECTOR'S INITIALS CLK <br />,r <br />t OBSERVATIONS <br />The following observations were made during the placement of soil gauges and the placement <br />of overburden on the tailings in the pond. <br />1) 7'he gauges placed on top of the geogrid and tailings consist of three standpipes welded <br />to metal plates, held in place by sandbags. A wire with a flag attached has been placed <br />w~_thin each of the pipes. Measurements are taken using a surveyor's transit scope and a <br />yardstick from the top of the metal wire. A static reference point was painted orange on <br />top of the interim dam where the underlying material was bedrock. The difference in <br />settling was found after the difference between the current wire height was subtracted <br />from the height of the transit's scope at the reference point on the interim dam. <br />2) The geogrid layer was left loose on the interim dam side, and at the north and south sides <br />of the tailings layer, and was fastened to the dam's geogrid layer with plastic ties. <br />3) .the ore abandoned on the west side of the mill was used as consolidation material for the <br />tailings; all of it was removed from the pile where it had been previously stored over to <br />the dam, where it was then placed on the tailings. <br />4) A hydraulic excavator arrived at 11:15 a.m. Placement of the borrow began at 3 p.m., <br />after all of the material had been moved within easy reach on the dam crest. Using a <br />toothless bucket, the dirt was placed on the tailings in layers of at least 1E" <br />Standing water in the tailings was removed as much as possible, before the borrow was <br />placed on top. <br />The results of the consolidation monitoring are to be faxed to DMG prior to any further work <br />on 'the tailings layer or dam, or any placement of liner, takes place.
