(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_~vRS_n9g <br />INSPECTION DATE: 7.1.n~/n~ INSPECTOR'S INITIALS: ~A!? <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology's four-year monitoring <br />program. Present at the inspection were Kate Pickford, DMG, and Marcia Talvitie, Four Corners <br />Materials. Weather at the time of the inspection was clear, calm, and approximately 70 degrees. <br />2. Disturbance at the site consists of an excavation at the crest of a hill. Most of the excavation is <br />encircled with undisturbed materials and stockpiled materials, including a topsoil stockpile. Disturbance <br />at the site is clearly not recent, as most of the disturbed areas are covered with vegetation, including <br />some well established Cottonwood trees. Ms. Talvite stated that no disturbance has occurred at the site <br />since before the Succession of Operator was approved in 2001 <br />3. According to Ms. Talvite, Four Corners Materials does not intend to continue mining at the site. In that <br />no mining has occurred at the site since at lease 2001, the site should be placed into a status of <br />Temporary Cessation, unless Four Corners Materials intends to perform reclamation activities at the site <br />within one year. The topsoil stockpile at the site is devoid of vegetative cover. Again, unless Four <br />Corners Materials intends to use this topsoil for reclamation within the next year, the stockpile should be <br />seeded to establish a vegetative cover. <br />4. Although the site could be reclaimed as a depression that may serve as a stock pond, if the proper <br />registration of the pond is completed with the Division of Water Resources, or as a swale, the landowner, <br />Mr. Wallace, arrived onsite during the inspection and stated that he would like to have the site leveled <br />off and reclaimed as a building lot for a house. If Four Corners Materials decides on this change in post- <br />mining land use, the current requirement for revegetation of the site would no longer be necessary. In <br />order to change the post-mining land use, however, Four Corners Materials would need to apply for an <br />amendment to the permit. <br />5. No bond calculation was conducted as a part of this inspection in that Four Corners Materials does not <br />intend to cause further disturbance a the site and intends only to reclaim the site.
