• (Page 21 <br /> <br />MINE ID JJ OR PROSPECTING ID k M-1980-055 <br />INSPECTION DATE 12/19/01 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Hard rock and Metal 110 permitted sites. The <br />operator was contacted but was not present during the inspection. The site was inactive at the time of the inspection. <br />The locked gate at the portal pad area contained a permit ID sign from the previous operator (EFN) but not from the current <br />permittee. Please refer to Rule 3.1.12 when preparing the sign for this and other sites to ensure that all required <br />information is included. The permit boundary corners were not marked. The lack of the sign and markers is noted as a <br />problem in this report. Please see the last page for tfie correction date. <br />The pad area contains a metal shop building, fuel and oil storage area, transformer pedestal in a fenced enclosure, material <br />storage area, etc. There are also two additional structures onsite not noted previously: a 40' semi trailer outfitted as a <br />change room, and a wooden shed on skids. <br />The oil storage area is not secured against damage due to accidental spillage: it is outside and on a soil surface with no <br />secondary containment. Several barrels contain oil, a couple barrels are tipped at an angle, and some barrels are open or <br />can be opened manually. Due to snow cover, it is unknown if spillage and soil contamination have already occurred. Given <br />the circumstances, the potential for contamination from Door hydrocarbon management is enough to consider this a <br />problem These barrels must be stored properly or removed Anv contaminated soil must be disposed of properly Please <br />see the last page for the correction date. <br />The portal is grated, but the grate has been cut allowing access by unauthorized persons. This is not noted as a problem, <br />but is being communicated to the operator so that better safeguarding may be implemented. It is recommended that the <br />portal be secured. <br />The maps are out-of-date, since they do not depict all structures, hydrocarbon storage areas, the proper topsoil storage <br />area, or access road corridor (part of which is clearly within the original permitted boundary!. The lack of completing the <br />intended map improvements in 1998 is now considered to be a problem. Please see the last page for the correction date. <br />There were no additional items observed. All problems must be corrected by the dates shown on the last page.
