(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~_2f10?- ~~ <br />INSPECTION DATE: ~-~-nd INSPECTOR=S INITIALS: inn <br />OBSERVATIONS <br />This was gore-operation inspection conducted in response to Leone Sand and Gravel LLC's 112 application for <br />the c~roposed Leone Pit. The application is for a proposed sand and gravel operation about b miles east of <br />Trinfcfad on land owned by frank L. Jr. and Shopie Leone. the sffe is about Yz mite south of hfahwav 160 and 350. <br />The company's consultant Joe Gagliano. of L J Development. Inc. and Frank and Shople Leone accompanied <br />me <m the inspection. <br />The south and west proposed permit area boundary lines are marked by a fence. Ocher boundaries will need to <br />be marked upon issuance of a permit. <br />A dry drainage bisects the area. The mining plan should Include adequate plans to keep runoff from earning <br />sediment to the drainage which would contribute flows to the Purgatoire River, a short distance to the north. The <br />applfcaffon shows fhat the drainage bottom is excluded from fhe proposed permit area. if also shows that the <br />operator plans to construct a berm for sediment confrol at the boundary between the operation and the <br />drainage. The berm will need to be constructed in such a manner that it will be effective as a sediment confrol <br />strw-ture. <br />A~~rtion of the southeast part of the proposed permit area, in Phase 1, was previously mined and reclaimed <br />The application Exhibit S only mentions fences as significant man-made structures within 200 feet of the affected <br />Ian<f. However, Iwo Colorado Interstate Gas Company pipelines cross the middle of the proposed permit area <br />in ttse same easement in asouthwest-northeast direction. The pipelines must be listed in Exhibit S and <br />addressed accordingiv. Documents in the application file show that CIG was contacted and that the gas <br />cornpanv reviewed the proposed plans for the gravel operation and sent a letter to the operator with <br />re~c uirements regarding the proposed mining operation in regard to their lines. Mr. Gaaliano was Informed that <br />a n afarized signature of a Leone Sand and Gravel officer on a copy of the letter agreeing to the requirements of <br />CIG would be required to satisfactorily complete Exhibit S. <br />The access road to the proposed permit area is the same access road that provided access from Hiahwav 160 <br />8 3:50 to the Latuda Gravel Pit. The road is on property belonging to the Leone's and they want the road to <br />rerrsain upon final reclamation of the site. I told the operator and Mr. Gaaliano that, if the road is to be <br />s~siflcantly upgraded, It would have to be added to the affected land area of the application. <br />(terns of concern noted above will be Included in an application adequacy review letter and they will have to <br />be addressed by the applicant prior to a decision on the application. <br />NAME: Joe Gaaliano <br />OPERATOR: L J Develapment.
