• (Page 41 • <br />MINE ID # OR PROSPECTING ID # M$7-038 <br />INSPECTION DATE 4/16196 INSPECTOR'S INITIALS WHE <br />The following problems IPBI andlor possible violations IPVI land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMSIPOSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Alignment of the mine entrance road has been altered from [he plans proposed on the Mine Plan Maps due to construction of <br />office/shop building. All sections of the entrance road were located within areas approved for road or for processing facilities. <br />No off site activity or environmental degradation has resulted from the realignment of the entrance road; however, the location <br />of the entrance road needs to be accurately portrayed on the Mine Plan Map, as required by Rules 6.4.3161 and Igl. Revised Mine <br />Plan Map shall also show the location of the relocated gas line at the north end of the Phase 1 lake area. <br />CORRECTIVE ACTION: Submit revised Mine Plan Map, showing the name and location of creeks, roads, buildings, oil and gas <br />wells and lines, and power and communication lines on the area of affected land and within 200' of all boundaries of such area, <br />as required by Rule 6.4.3161 and Igl. Revised Mine Plan Map, showing the locations of the entrance road and relocated gas line <br />may be submitted as a Technical Revision or may be included in the Amendment proposed by the operator. <br />6122/96 <br />PB or PV PB <br />TOPICISI RD <br />MP, , <br />Additional perimeter markers are necessary along the NW boundary, which forms the west boundary of Phase 5 area, to ensure <br />operations remain within the approved affected area. Questions were raised during the inspection regarding the removal of an <br />island of material remaining at the SW corner of Phase 5 area. DMG inspector could not determine whether the referenced island <br />of materiel was located within the approved affected area due to absent perimeter markers. Rule 3.1.12121 states that the <br />boundaries of the affected area will be marked by monuments or other markers that are clearly visible and adequate to delineate <br />such boundaries. <br />CORRECTIVE ACTION: Post and maintain perimeter markers for the entire affected area, as required by Rule 3.1.12(21. <br />6/22196 <br />PB or PV PB <br />TOPICISI SM <br />3. Mobile fuel tank was located adjacent to the generator trailer. Fuel tank appeared intact, no evidence of rupture or leaking was <br />observed; however, spill preventionlspill control structures for the fuel tank were not evident. Fuels, lubricants and chemicals <br />need to be stored in a controlled manner, et a designated location, with adequate spill prevention and spill containment controls <br />to ensure disturbance to the prevailing hydrologic balance be minimized as required by Rules 3.1.6 and 3.1.7. Spill containment <br />controls typically include measures such as construction of a containment berm surrounding fuel tank or fluid storage areas. Berm <br />surrounding the fluid storage area should enclose sufficient area to ensure containment of volume equivalent to tank capacity. <br />Containment areas should be lined with an impermeable liner to ensure spilt fuel or fluids not escape end degrade quality of <br />surface and groundwater (Rules 3.1.6 and 3.1.71. <br />CORRECTIVE ACTION: Ensure protection to the prevailing hydrologic balance, required by Rules 3.1.6 and 3.1.7, by constructing <br />spill containment structures for all fuel, lubricants and chemicals stored on site, OR remove fuel, lubricants and chemicals from <br />permit area. 6/22196 <br />PB or PV PB <br />TOPICISI HB <br />AT, SF, _ <br />Certified Mail No. Z 287 172 910 <br />Return Receipt Requested <br />
