(Page 2)

MINE ID # OR PROSPECTING ID # M-79-102
INSPECTION DATE 6 1 98 INSPECTOR'S INITIALS ~I I$
OBSERVATIONS
The following observations were made as part of a routine inspection of this 9.9 acre 110
gravel pit.
1. A sign with the Operator's name and DMG permit number was posted at the entrance to the
site, whose boundaries are defined by natural barriers, fencing and stakes.
2. This two-tiered site was paced out (due to mud) and is approximate 200 paces (270')
running east-west, and approximately 500 paces (670') north-south. The lower tier is bounded
along the north side by highwalls and 1H:1V slopes rising approximately 20', and the upper
tier is bounded along the north side by 1H:1V slopes rising about 30'.
3. Topsoil is stored along the north boundary in a stockpile approximately 1'-2' deep and 30'
across.
4. Oil tanks on the site have sufficient berming to prevent any spills.
5. A fines stockpile is stored along the lower eastern boundary; it is approximately 15'
across, 5.5' high, and runs approximately 200' along the boundary.
6. Aspens along the edges of the site have not been disturbed at this time.
No problems were apparent at this time. The bond will be recalculated and the financial
warranty checked within a month to determine if the financial warranty is still current.
Should the Operator wish to reclaim the site to something other than the current designated
end land use, a technical revision with amendment notifications must be filed with DMG prior
to any changes in the reclamation work.
I & E Contact Address
NAME Philip George
OPERATOR Cross Mountain Ranch.
STREET Box 597
CITY/STATE/ZIP Craig.
