(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-ie9~-n~9 <br />INSPECTION DATE: (L/26/0'~ INSPECTOR S INITIALS: KAP <br />OBSERVATIONS <br />1. This inspection was conducted at the request of the operator for clarification as to what actions are <br />necessary for approval of two Technical Revisions (TR) currently under review at the Division. This inspection <br />report will address both portions of the inspection related to the two separate TR's (TR-004 and TR-005). Present <br />at both portions of the inspection were Kate Pickford, and Carl Mount, DMG, and Connie Davis and Mike <br />MacAfee, Aggregate Industries -WCR, Inc. Weather at the time of the inspection was clear, breery, and <br />approximately 65 degrees. <br />2. The first portion on the inspection addressed issues related to TR-004, which concerns further measures <br />Aggregate Industries must take related to a fuel spill that occurred at the site in 2001. In addition to the parties <br />listed above, present at this portion of the inspection were Jamie Gaboriau, Aggregate Industries -WCR, Inc. <br />and Scott Rutherford, Paragon Consulting Group. Currently, Aggregate Industries has committed to removal of <br />all of the potentially contaminated materials that remain at the site of the fuel spill prior to cessation of <br />dewatering at the site. Analysis of the site by Paragon indicates that the remaining materials do not pose the <br />threat of contaminating groundwater once dewatering at the site ceases and groundwater contacts the <br />material. Therefore, TR-004 requests that Aggregate Industries be released of the requirement to remove the <br />remaining materials. <br />3. The Division has expressed concern that despite the results of Paragon's investigation, when dewatering <br />ceases, monitoring at the site should be continued to confirm the validity of Paragon's model. Therefore a plan <br />to sample the remaining monitoring wells plus one additional well on a weekly basis once dewatering ceases, <br />until the wells are below the water level in the pit was discussed. The Division believes that the following <br />parameters should be included in the analysis during this monitoring phase: benzene-toluene-ethylbenzene- <br />xylenes (BTEX), total volatile petroleum hydrocarbons (TVPH), and total petroleum hydrocarbons in the diesel <br />range. Aggregate Industries will need to submit this monitoring plan as an addition to the TR-004 request for <br />review prior to approval. <br />4. The second portion on the inspection addressed issues related to TR-005 that proposes installation of a <br />stormwater culvert by the City of Greeley, which drains into a groundwater lake that will be present on the site <br />once dewatering of the pit ceases. In addition to the parties listed under item 1, present at this portion of the <br />inspection were William Schenderlein, Applegate Group, John Briggs, City of Greeley, and Tom Burnett, Burnett <br />Consulting Engineers. This portion of the inspection addressed issues thaf were unclear in the TR-005 <br />application. Primary issues that were identified regarding adequacy of the permit applications involved <br />specifications related to culvert design, parameters used to calculate projected inflows, and the capacity of <br />the existing outflow from the future groundwater lake. <br />5. It was determined that all of the information in question is contained in a report for the City of Greeley <br />composed by Burnett Consulting Engineers.
