(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-i9Rn-9dd <br />INSPECTION DATE: in-~n_n~ INSPECTOROS INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the Phase I, II III AND IV areas on <br />10-2-03. Present during the inspection were, Phil Barnes and Mark Tidquist of CCBV. Inspected the ARD <br />additional SPILL Containment area. The inspection began around the Phase IV pregnant solution collection <br />area. The recently deployed pad was being leached at the time of the inspection. The following readings were <br />taken from the Phase IV, high and low volume solution collection transducers. Low volume solution collection <br />transducer # 1 was recorded at 7.2 # 2 was at 2.4. (All readings are In inches of hydrostatic head). High volume <br />collection transducer # 7 was recorded at 14.6 # 8 was of 15.8 and #9 was at 16.4 (All readings were in feet of <br />hydrostatic head). <br />Inspected the recently deployed ore that was being leached at the time of the inspection, according to Phil, the <br />additional work force hired to plow the upper portion of the pad to facilitate added percolation is progressing <br />well, no major problem was noted. CC&V is still utilizing a D6 to furrow the upper layer for Increased percolation. <br />He also stated that the CC&V is working diligently to find a permanent solution. Inspected the ARD ADDITIONAL <br />SPILL PREVENTION AREA, NO MAJOR PROBLEM NOTED. <br />Inspected the external pond, it was still holding some water, no problem noted. According to Phil, as soon as it <br />(the water) Is needed, it will be pumped back onto the pad as make up water. While in the area, the following <br />readings were recorded from the low volume solution collection transducers. Transducer # 1 was recorded at <br />11.4 and # 2 was at 13.3 inches of hydrostatic head. Pond level was of 13.5 feet <br />Inspected the Phase I and II High and Low volume solution collection systems and recorded the following <br />readings from the transducers. Phase I, High volume solution collection transducer # 1 was recorded at 42.2 # 2 <br />was at 44.4 and # 3 was at 44.3. Piezometer was at 46.9. Low volume for Phase I was recorded at .22 and # 2 <br />was at .35 (ALL READINGS ARE IN FEET OF HYDROSTATIC HEAD) <br />Phase II High Volume Solution Collection transducer # 4 was recorded at 23.7 #5 was at 13.6 and # 6 was at <br />21.8. Piezometer was recorded at 28.7 Low volume solution collection transducer # 1 was recorded at .15 and # <br />2 was at .71. (All transducer readings unless noted are in feet of Hydrostatic Head). Inspected the under drain <br />area, both the south and 4 Inch pipes were discharging at the time of the inspection, the north under drain was <br />still dry.
