ORIGINAL • PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_toRn-gee <br />INSPECTION DATE: I~-~~-n'~ INSPECTOROS INITIALS: ~lC- <br />OBSERVATIONS <br />The Division conducted and construction progress inspection of the Phase <br />IVC area and a monitoring inspection of the existing leach pads on <br />04/22/04.Phase IVC leach pad construction commenced on 4/16/04. Ames <br />construction, the sub contractor constructing the leach pad had mobilized <br />all the needed earth moving equipment to the site. Presnt during the <br />inspection was Mr. Phil Barnes of CC&V. inspected the grading operation <br />along the North and south sides of the Phase IVC leach pad area. Only two, <br />dozers were working at the time of the inspection, no major problem noted. <br />Inspected the Phase I, II, III AND IV LEACH PAD and Pregnant solution areas <br />and collected the following readings from the High and Low Volume solution <br />collection transducers. <br />Phase I High Volume solution collection transducer #1 was recorded at 35.7, <br />#2 WAS AT 38.3 AND # 3 WAS AT 32.8. PIEZOMETER FOR THE POND WAS AT 37.4 <br />FEET. LOW VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS RECORDED AT .27 AND <br />#2 WAS AT .71. <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT <br />41.3 # 5 WAS AT 38.3 AND # 6 WAS AT 43.6. LOW VOLUME SOLUTION COLLECTION <br />TRANSDUCER # 1 WAS RECORDED AT .13 AND # 2 WAS AT .27. Piezometer for the <br />area was at 42.8 feet. <br />Phase IV High Volume Solution collection transducer # 7 was recorded at <br />19.2 # 8 was at 19.2, # 9 was at 19.3 and # 10 was at 19.7. Low Volume <br />solution collection transducer # 1 was at 18.6 and # 2 was at 5.9 inches of <br />hydrostatic head. (ALL transducer readings unless noted, are in feet of <br />hydrostatic head) <br />Inspected the external pond, it was holding partially thawed water. <br />According to Phil the water is expected to be used as make up water as soon <br />in the next few weeks. While in the area, collected the following readings <br />from the low volume solution collection transducers.
