(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1978-085 <br />INSPECTION DATE 07/12/2006 INSPECTOR'S INITIALS DEG <br />OBSERVATIONS <br />This routine monitoring inspection of Permit Number M-1978-085 (Smith Pit #20) was conducted by Deb E. Gbnima of the <br />DMG. Moffat County representative Marvin Moore was present during this inspection. The pit is located 49 miles north of Craig, <br />CO on the north and south sides of County Road 4, and currently consists of approximately42.1 acres where approximately 25 <br />acres have been disturbed. The northern permit boundary is the Wyoming/Colorado state line. <br />Although the permit boundaries cross County Road 4, no disturbance has occurred on the south portion of the permit area yet. <br />Mine entrance signs and permit boundary markers are all in place, and topsoil stockpiles are all well-marked. All activities and <br />completed work at the time of inspection appeared to follow the guidelines and regulations set forth by the state and the mining <br />and reclamation plans for the pit, and no problems are noted for this site. The site is stable and no disturbance was observed <br />outside of the approved permit boundaries. For future reference, this site is correctly positioned in the DMG online GIS. <br />The eastern portion of the site was reclaimed (sloped, topsoil reapplied, and seeded) a few years back, and while stable, may <br />need a few additional years for vegetation to become well-established. The storm water runoff control program that was <br />implemented a few years back seems to have been successful in its use of diversion structures, sediment traps, and small <br />ponding areas. There appears to be no offsite erosion, and no long-term (greater than 72 hours) water retention was observed. <br />The only items of concern were the presence of an asphalt stockpile (see photo 3) and a fuel tanker truck onsite without the <br />proper secondary containment measures (see photo 4). However, it was explained that the tanker was only onsite to <br />temporarily service the road maintenance operations going on nearby (which we passed on our way to the site), and that once <br />these operations were concluded (in less than 2 weeks) the tanker and the asphalt would be removed. <br />Responses to this inspection report should be directed to Deb E.
