• (Page 2) '- MZNE ID # OR PROSPECTING ID # N-94-117 INSPECTION DATE 5-22-97 INSPECTOR'S INITIALS GC OBSERVATIONS This was a monitoring inspection conducted to monitor the status of the spray evaporation and land application systems and the Hazel-A adit. No one was on-site for the operator. Tailings Pond There was approximately 17 inches of freeboard in the tailings pond. This was determined by observing the pond water level on the graduated marker taped to the decant riser. An additional 6 inches was added to that level to account for the freeboard above the top of the riser. graduated marker reading = 11 inches added freeboard amount = 6 inches Total 17 inches Tailings Pond Spray Hvaporation System The :;pray evaporative system was not operating during this inspection. This may be a violation of the permit (see Possible Violation PV-1). Water was being pumped from the Hazel-A adit into the pond via a bypass in the pvc line. In all, we counted 58 sprinkler heads spaced uniformly along the two long sides of the pond. LAND APPLICATION SYSTHN The Land Application system is outlined in an April 11, 1996 letter from Mr. Fox. regarding `water Spray Land Application System, Gold Hill Mill.' This system has been installed. HAZHL-A Adit The H,szel-A adit was inspected and the freeboard behind the coffer dam bulkhead was measured at 22." No inflow was observed through the shear zone in the adit roof between the bulkhead and the portal. The sumps outby of the bulkhead were relatively dry.
