(Page 2) <br />MINE ID # OR PROSPECTING ID # M-78-200 <br />INSPECTION DATE 5/1/98 INSPECTOR'S INITIALS AJW <br />OBSERVATIONS <br />This was a normal monitoring inspection conducted by Tony Waldron of DMG. The operator chose <br />not to accompany the inspector during the inspection. The pit is located approximately 10 <br />miles west and 1 mile south of Pritchett, Colorado and is adjacent to County Road 2. <br />The legal description for this permit application coincides with another application (M-78- <br />225) on file with our office. Based on the physical inspection of the property it was <br />determined that the two permitted areas (M-78-200 & M-78-225) are in fact the same site. <br />There was an identification sign posted at the mine entrance which had the M-78-225 number <br />on it and the affected area boundaries were clearly marked. Topsoil was being properly <br />salvaged and stockpiled for later use in reclaiming the site. <br />After the inspection was completed and the situation of having two permits for the same site <br />was discussed with the operator, it was jointly decided that this application (M-78-200) <br />should be terminated and File M-78-225 would be retained as the active working permit for <br />this mine site. In order to complete this action, the operator must send a letter to the <br />Division requsting that File M-78-200, which is located in the NW 1/4 of the SW 1/4 of <br />Section 9,~Township 31 S, Range SO W of the 6th P.M., be terminated and replaced by Permit <br />No. M-78-225.
