(Page 2) <br />MINE ID # OR PROSPECTING ID #: mod-X991-14!>- <br />INSPECTION DATE: 0_4/»/.06 INSPECTOR'S INITIALS: KAP <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology's routine monitoring <br />program of in-stream operations, and also served as training for newly hired EPS's with the Division, <br />regarding in-stream gravel operations. Present at the inspection were Kate Pickford, Wally Erickson, <br />Chris Rowe, Deb Gonima, and Rick Wenzel, DMG, Robert Larson, Monadnock Mineral Services ,and <br />Travis Sackman, ZMK Mine Construction, Inc. Weather at the time of the inspection was clear, breery, <br />and approximately 60 degrees. <br />2. This inspection occurred prior to the peak of 2006 spring run-off. At the time of the inspection, the levee <br />between the operation and the main channel of the Uncompahgre River was intact. The operator <br />does not plan to allow the river to enter the site this year, and indicates that he believes there is enough <br />material at the site to allow production for the upcoming year, prior to requiring recharge from the river. <br />3. The permit boundaries are marked. The pit, resulting from excavation at the site, is immediately <br />adjacent to the south boundary of the permit. Disturbance south of this permit boundary, in the form of <br />heavy equipment disturbance, is related to maintenance of the levee, and not to mining activities. <br />4. The levee upstream of the site has been moved out further into the river channel and has been <br />reinforced relative to the previous inspection. Maintenance of these levees is the responsibility of the <br />individual land owners. <br />5. Activity at the site is in compliance with the permit and with the Construction Materials Rules and <br />Regulations.
