The following observations were made during the inspection.
1. No standing pools of groundwater appeared to be exposed. Pictures were taken of water in
the pit, but it appeared to be the result of runoff from the recent snow.
2. A new traneect of groundwater monitoring etatione has been installed since the last map
(Figure D-2) was submitted to DMG. A new, updated copy should be sent to us ae soon ae
possible. The etatione appear to be accessible and placed in points necessary for accurate
data collection.
3. The boundaries of the new and old amendment areas were marked with visible pink ribbon
markers. Fencing was color-coded, with normal boundary fences painted brown, and wetlands
fencing painted white within green fences. The mapping of the new amendment area appeared
to be accurately reflected on the map.
4. Grading and tapering of the channel was in conformance with the mining and reclamation
plane. Areas where revegetation had begun were fenced off. Some small areas needed finish
and elope grading, but these were not more than 20 feet long.
Overall, the site appears to be in compliance with its current permit, and no problems with
the proposed reclamation and mining plane were noted.
I & E Contact Address
NAME Garv Tuttle
OPERATOR Triad Western Corporation
STREET 1190 Grant St..
