(Page 2) <br />MINE ID # OR M-2002-109 INSPECTION DATE: 7/26/2006 INSPECTOR'S CMR <br />PROSPECTING ID #: INITIALS: <br />OBSERVATIONS <br />This inspection was carried out at the request of the Operator Mr. Anthony Blasi to determine if an acreage <br />and/or warranty reduction is warranted. Chris Rowe from the Division Reclamation Mining & Safety (Division) <br />carried out the inspection; also present was Mr. Anthony Blasi. The mine is located west of Cucharas <br />Reservoir. The land is owned by the State of Colorado and managed by Colorado Land Board. <br />This is a ridge top sand and gravel mine. An upgraded two-track road is used to access the mine site. The <br />road is approximately 20 feet wide and 4600 feet long. The road goes over one ephemeral stream crossing; a <br />culvert is in place to convey the water. Mining operations have removed approximately 8-10 feet off the top of <br />the ridge. The site is relatively flat. The southern half of the ridge has been mined, regraded, topsoiled, and <br />revegetated. Seeding occurred in the spring of 2006. Grasses and (orbs were becoming well established. <br />There were a few bare areas, but in general the reclamation was progressing according to the reclamation <br />plan. The northern half of the site has been mostly mined out. Grading is completed on most of the northern <br />half, with some stockpiles and an area still to be mined at the northern edge. The topsoil was limited, but there <br />was not apparently much topsoil to begin with. Given the revegetation success in the southern half the lack of <br />topsoil does not appear to be a problem. <br />There were not any problems noted at the site. <br />While the revegetation efforts are making good progress, the southern end of the mine site is not yet ready for <br />release, but could be within one to two years. The Division carried out a surety calculation. The estimated <br />amount to reclaim the site is $14,151. The Operator may request a warranty reduction by following Rule 4.14.1. <br />4.14.1 Operator's Request for Reduction <br />(1) An Operator may request that the Office reduce the amount of the Financial Warranty required. <br />(2) Such a request must: <br />(a) be made in writing, separate from other correspondence; <br />(b) include an estimate of the actual cost to reclaim the site based on what it would cost an <br />independent contractor to complete reclamation, including unit costs for reclamation activities as <br />appropriate to the operation to comply with the provisions of Rule 3.1 and the Permit's <br />Reclamation Plan. <br />Enforcement and Inspection Contact Address <br />NAME: Anthony Blasi <br />OPERATOR: A.
