w
~~
Stormwater - -Industrial Activity
Field Audit /Inspection Form (page 2 of 2)
Date: 10/11/95 Inspectors: Daniel Beley, Bob Griffith Cert.No.
COR-040073
Facility Name: Gold Hill Mill
E. Vehicle maintenance or fueling; storage of fuel? yes no X
Observations:
F. Evidence of other exposed industrial activities? yes no X
Observations:
G. Evidence of non-stormwater discharges'! yes X no
Observations: Tailings pond can drain to the Hazel A Adit. There was evidence of a discharge from the Hazel A Adit
observed on the day of the inspection.

H. Best Management Practices, installation, maintenance, effectiveness; additional comments:
-A dire. berm exists at the toe of the tailings pile just southwest of the mill building. Stormwater collected behind this
berm evaporates or is diverted to the tailings pond via a french drain. This berm had been damaged during a recent
move of a trailer, and had no[ been repaired at the time of the inspection. This berm needs to be restored so [hat is
able to function effectively.
-Waste: oil drums were stored outside of the loading/unloading area on the northeast side of the mill building. Soil was
stained in this area and evidence existed that would indicate that this spilled oil was migrating away from [his area. No
Best Management Practices were evident to mitigate [his risk.

10/93/INSI~CT I.FRM
