~, <br />(Page 2) <br />MINE ID f OR PROSPECTING ID f M-94-117 <br />INS PIiCTION DATE 8/26/95 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />Present for this inspection were Mark Steen, who owns land within a portion of the permit <br />area as well as adjacent land, and Bob Mason, a resident of Gold Hill, and a representative <br />of the Committee on Mining and Environment within Gold Hill Town Meeting, inc. <br />Tail:inc Pond Liner <br />Patches had been applied in several locations on the liner since the last inspection which <br />was conducted on 7/19/95. The patches generally consisted of thin strips of HDPE placed over <br />damaged areas and liestered (hot air device) to the existing liner. This type of patching <br />is not acceptable under the terms of the permit. It was observed that the patches could be <br />removed with little or no effort. All liner flaws and defects must be repaired using an <br />extrusion welding apparatus. All repairs must be vacuum tested. The operator must provide <br />the Division with a set of technical specifications detailing the repair and vacuum testing <br />procedures to be employed. All repairs must be completed by November 1, 1995. <br />it w:)s agreed that new 30 mil HDPE would be welded to the existing liner at the tailing dam <br />crest, such that existing defects will be cut out of the existing liner or covered by the new <br />liner, and such that there ie sufficient new liner attached to completely seal the upstream <br />face of the dam and moat of the dam treat, and such that the liner can be adequately anchored <br />or covered on the dam treat. This will be completed by November 1, 1995. <br />Attaching the strip of new HDPE liner at the tailing dam crest will require removal and <br />resetting of the existing fence posts. The fence must be reinstalled in a workman like <br />manner such that the poste are well set, using concrete if necessary, and the fence ie tight <br />and secure. The poste should be installed near the outside edge of the dam crest in order <br />to maximize coverage by the HDPE liner. <br />A major liner flaw that exists ie at the top of the back of the tailing pond. A tree has <br />grown up at the edge of the liner at this location. It was agreed that a new piece of 30 mil <br />HDPE would be welded on to the existing liner at this location, and that the liner would be <br />reinstalled in an anchor trench of the dimensions specified in the permit. <br />storm Water controls <br />The outlet of the small sediment pond below and between the tailing pond and the mill <br />building ie becoming eroded. It was agreed that the operator would armor the pond outlet <br />with rip rap. This must be completed by October 1, 1995. <br />The cliveraion ditches above the tailing pond are silting in at some locations. The operator <br />agreed to clean out any silt or debris in the ditches that would be an impediment to flow, <br />and t:o trim the vegetation in the ditches. This must be completed by October 1, 1995. <br />General Site Condition <br />The operator agreed to clean up all junk and equipment that ie scattered around the site. <br />Any trash must be removed to an off-site landfill. Any usable equipment moat be consolidated <br />into a single organized bone yard near the mill building. Any chemicals or petroleum <br />products moat be stored inside the building; empty barrels must be removed from the site and <br />properly disposed of. site clean-up must be completed by October 15, 1995. <br />Oil eiaturated soil was observed around the compressor and vacuum pump housed in a lean-to at <br />the Lrphill end of the mill building. This contaminated soil must be excavated and properly <br />disposed of off-site by November 1, 1995. <br />Haze].-A Adit <br />Several small leaks from the Hazel-A bulkhead were observed. There was a small discharge <br />from the Hazel-A ad it to Cash Gulch at the time of this inspection. The water level was <br />approximately 20" below the top of the bulkhead. No water has been pumped from the Hazel-A <br />up to the pond this summer, however, by the time this report is being written (9/5/95) the <br />operator has reported that pumping of Hazel-A water to the pond was initiated August 30, <br />1995. It was agreed that the operator would install a lined coffer dam and pump-back system <br />outb}• of the bulkhead to prevent discharge from the Hazel-A. This system moat be in place <br />by October 1, 1995. It was agreed that if the coffer dam ie not effective in eliminating <br />discharge from the Hazel-A, that a concrete cut-off wall would be installed and keyed into <br />bedreck. If the concrete cut-off is needed, it would have to be installed and operational <br />by Ncvember 1, 1995. It ie imperative that all water be purged from the Hazel-A, through <br />
