(Page 3)
MINE ID # OR PROSPECTING ID # M-1982-015
INSPECTION DATE 04/11/2006
INSPECTOR'S INITIALS DEG
16. Please provide clarification of potential solutions to permeability issues arising as a result of the intersection of the
utilities' pipes (i.e. the water main in the berm between current ponds 2 and 3, also any gas and sewer lines) and the
proposed slurry wall.
17. As discussed during this inspection, please provide documentation describing how the operator will address the
apparent lack of suitable fines on site for slurry wall construction. The requirement is between 20°k and 50% fines for
the slurry mixture, and the operator states that present site conditions provide merely 24% to 28%.
18. Please provide additional information regarding the location and source of borrow to be used on site.
19. Please provide an explanation as to the use or disposal of the cement stockpiles found on site during the inspection.
20. Please explain how the asphalt road at the south edge of the ponds (between the main pond and the St. Vrain) will be
reclaimed.
21. Please provide the acreage of the permitted area and clearly illustrate the size and location of the temporary sediment
ponds on a map.
22. Please provide documentation revising the grade of the swimming area from 3h:1v to 5h:1v as required by Rule
3.1.5(7).
23. There appears to be a minor knapweed population on the southwestern side of the current middle pond. Please submit
a weed control plan per Rule 3.1.10(6).
24. Please choose one of the three options for calculation of the slurry wall bond from the information provided by DMG
during this inspection. The current bond held is $46,800.
25. Please provide additional detail for revegetation techniques that will be implemented - i.e. mulching (how much/acre
and with what) and fertilizer (how much/acre and with what).
This list was compiled as a courtesy to the operator so that information can be gathered. However, DMG would appreciate it if
these items were not submitted until after the adequacy review letter has been received by the operator.
As a courtesy DMG will also provide a recommended seed mixture with the adequacy review letter, that the operator may
choose to utilize at his or her discretion.
Responses to this inspection report should be directed to Deb E. G6nima at the Division of Minerals and Geology, Denver
Office, 1313 Sherman Street, Room 215, Denver, Colorado 80203, phone number 303-866-4179.
I & E Contact Address cc:
^ CE
NAME _ ^ BL
OPERATOR _ ^ FS
STREET _ ^ HW
CITY/STATE/ZIP _ ^ HMWMD (CH)
^ SE
^ WOCD (CH)
X OTHER (Carma and Siegrist)
cc: Tyler Packard and Brad Davis, CARMA
Mr. Mike Siegrist
Carl Mount, DMG via a-mail
