i, <br />• (Page 2j <br />MINE ID ~ OR PROSPECTING ID ¢ M-94-117 <br />INSPECTION DATE 12/11/95 INSPECTOR'S INZTIALS ACS <br />OBSERVATIONS <br />Mark Steen, who owns land within the permit area ae well as adjacent land, and Bob Mason, <br />representing the Town of Gold Hill, were present for this inspection. <br />Tailing pond liner repairs were underway at the time of this inspection. Equipment in use <br />included a diet sander, leieter, and a hot air type extrusion welder. Since the hot air type <br />extrusion welder dose not have temperature control capability, welds completed using this <br />apparatus would not be acceptable for the impoundment of tailing, however, based on visual <br />inspection of the welds, the Division finds that they will be acceptable for the impoundment <br />of water, ae long ae vacuum testing is completed to demonstrate that the welds do not leak. <br />Prior to discharging any additional tailing to the pond, the dam would have to be raised in <br />accordance with the approved permit, and any welds completed with the hot air extrusion <br />welder would have to be capped using a temperature controlled extrusion welder. <br />During this inspection, several holes in the liner were identified along the upstream face <br />of ttie dam and elsewhere. It is reiterated here that all liner flaws and defects moat be <br />repaired using an extrusion welding apparatus, and that all repairs must be vacuum tested. <br />It ie the operator's responsibility to carefully examine the entire lined area and identify <br />any flaws that exist. The extrusion welds completed by In-Line Plastics, Inc. and the <br />extrusion welds completed by the operator to date, were visually inspected and generally <br />found to be acceptable. <br />The Fiazel-A adit was inspected. The Hazel-A bulkhead had been reconstructed subsequent to <br />the trailing slimes removal project. The joints between the bulkhead and the rib and the <br />joints between the bulkhead timbers had been caulked. The plastic liner on the inby aide of <br />the bulkhead still had to be lapped to the rib using a rubberized sealant. <br />The pump-back system outby of the Hazel-A bulkhead had been reestablished. The pump-back <br />hose was found to be punctured, and will have to be repaired. Also, it was noted that the <br />bracing for the decant system piping in the Hazel-A was over-stressed and needs to be <br />upgraded.
