IV. ENFORCEMENT ACTIONS <br />The following identifies enforcement actions taken during this inspection, the required <br />abatement and date of abatement for the violation, previous violations still requiring <br />abatement and the termination of violations cited during previous inspections. <br />NOV CU-2002-002 was terminated effective July 10, 2002 following receipt of a copy <br />of a certified receipt for the house at 20730 Highway 12. NOV CV-2002-009 was <br />issued on June 17, 2002 for failure to conduct surface blasting operations in <br />accordance with the Rules and Regulations and in accordance with the DMG permit. <br />The abatement date to submit a technical revision to the permit was set for July <br />8, 2002. On July 8 the abatement date was extended to July 9, 2002. The Division <br />on July 9, 2002 received a technical revision (TR-09) and the NOV was terminated <br />effective that day. The civil penalty has been assessed at $1,900.00 and the <br />operator has requested an assessment conference that has not been held as of this <br />time. <br />No enforcement actions were taken as a result of this inspection. <br />NOTE: Attach appropriate VIOLATIONS or CESSATION ORDER CODING FORMS <br />Revised 2/97 <br />m:\coal\share\forms\8101 <br />5 <br />
