IV. ENFORCEMENT ACTIONS
The following identifies enforcement actions taken during this inspection, the required abatement and date of
abatement for the violation, previous violations still requiring abatement and the termination of violations
cited during previous inspections.
No enforcement actions resulted from this inspection. A summary of the active
enforcement actions follows:
NOV CV-2000-002 was issued for failure to provide to the DMG a certificate of public
liability insurance policy necessary to certify that such policy is active, as required under
C.R.S. 34-33-11015) and Rule 2.03.9(1). During the time of this inspection, DMG verified
that copy of the current general liability insurance policy 04GL000021953, effective
12/5/99, expires 12/5/00, was contained within the mine records.
NOV CV-2000-003 was issued for failure to maintain copies of all records, reports, and
information for public availability at a convenient place in the area of mining, as required
under C.R.S. 34-33-122(8) and Rule 5.02.4(11. During the time of this inspection, DMG
verified that copies of the required records, reports and information is available and
maintained at the mine office.
NOV CV-2000-004 was issued for failure to provide the name, address, telephone number,
and SSAN of the applicants' resident agent, as required under C.R.S. 34-33-11011) and
Rule 2.03.4(21(b). During the time of this inspection, the operator provided an updated
phone number for Shawn Price, [phone].
