(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-iaRn-77a <br />INSPECTION DATE: a-s-m INSPECTOR'S INITIALS: ~~ <br />The Division conducted a monitoring inspection of the site as part of the fiscal year 2000-2001, sites to be inspected. Only <br />processing of mined material takes place on site, no mining is anticipated in the future. The processing area includes a <br />gravel wash plant with associated sediment ponds and a concrete batch plant. The site also includes an equipment <br />storage area, asphalt stockpile area. And aggregate stockpile area. Present during the inspection was Miss Angela <br />Bellantoni of Redi Mix. The entire permit area was driven and inspected, asphalt and processed aggregate were stockpiled <br />along the North side of the permit area. Both the asphalt and concrete batch plants were active at the time of the <br />inspection, no problem noted. Existing high calls were stable no major sign of stability was noted. <br />Along the East side of the permit area, the operator had constructed a new office building and parking lot, which is not <br />covered with the existing warranty. Miss Angela Bellantoni was notified that the Division had calculated the warranty for <br />the added disturbance to be $ 100,000.00, which she agreed to post with in 60 days. At this time, since the operator <br />agreed to post the warranty, the fact that the structure was placed prior to approval and posting of the appropriate <br />warranty will not be noted as a possible problem. A letter requesting the additional warranty was faxed and hard copy <br />sent to the operator on 4-10-O1.
