(Page 2J
MINE ID # OR PROSPECTING ID #: nn_~nns_ t~-
INSPECTION DATE: 3J9rn~ INSPECTORS INITIALS: -KAP_
OBSERVATIONS
1. This inspection was conducted in response to an acreage release request and an amendment
application. Present at the inspection were Kafe Pickford, DRMS, and Nick Gray, the
Applicant/Operator. Weather at the time of the inspection was clear, calm, and approximately 50
degrees.
2. The Applicant has submitted a request to remove 1.86 acres from the permit. This acreage was
included in the original application to allow for the installation of an access road at the site. Since the
permit application was approved, however, the Operator has received approval from the county for
an alternate access road, which lies within the existing permit boundary.
3. The acreage proposed to be released from the permit currently contains atwo-track which was to be
improved for use as the access road, but the existing road was present prior to permitting of the site.
The existing road has not been altered since the permit was issued, and none of the remaining 1.86
acres has been disturbed. Therefore, the acreage release request will be approved.
4. The amendment application addresses issues related to the new access road to the site. The public
comment period for the amendment application has not yet expired and a decision related to the
amendment application will be made upon further review of the application materials and dependant
on whether comments regarding the application are received by the Division.
I & E Contact Address Cc~ ^ BL
^
NAME: Nick Grav ^ FS
^ HW
OPERATOR: Nick H Grav ^ HMWMD (CH}
STREET: .Sfi4 Ffi 30 Road ^ SE
^ WOCD (CH)
CITY/STATE/ZIP: Mnntrtxa C'r~ R1401 ^ Other
