• • (Page 3) ~~ <br />MINE :CD # OR PROSPECTING ID # M-94-117 <br />INSPECTION DATE 9-5-97 INSPECTOR'S INITIALS GC <br />graduated marker reading 14 inches <br />added freeboard amount 6 inches <br />Total = 20 inches <br />Problem PB-1 in the a-21-97 inspection report required the operator to repair a torn area of <br />the pond liner. This area has been patched. The operator needs to submit verification that <br />the hole was repaired using an extrusion welding apparatus and that the repair was vacuum <br />tested. <br />Tailings Pond Spray Hvaporation System <br />The spray evaporative system was operating during this inspection. The large spray head was <br />also operating. The recirculating pump for the spray evaporative system has been set into <br />a sump area which has been dug into the tailings in the bottom of the pond. The foot valve <br />intake was measured at 24.5" below the pond surface and it is set in an open 5 gallon bucket. <br />This is still an inefficient screen and needs to be improved. The intake must be placed <br />fully enclosed within a screen inside a coarser screen or filter. This has been noted in <br />past inspections and will be discussed during the September Board hearing. <br />It wars also observed during this inspection that not all spray heads are flowing at full <br />capacity. The system must be operating efficiently and as approved. Therefore, the operator <br />must submit verification that this system meets the capacity as designed by Richard Fox and <br />approved by the Division. See Problem PB-2. <br />Portions of the spray line formerly along the downhill side of the pond are now fully on the <br />liner and the spray directed inside the lined pond. <br />Monitoring Wells <br />The operator has now secured the monitoring well protective covers with locks. <br />Other <br />Scrap pvc has been cleaned out of the diversion ditch near the pond as required in Problem <br />PB-3 of the 8-21-97 inspection report. <br />Use of the designation letter "Y" (Y=Inspected and Found in Compliance) in the GENERAL <br />iNS P]'sCTION TOPICS section of page 1 of this report is not intended to abrogate pending <br />problems and possible violations identified in previous reports filed for this site.
