Dear Mr. Dismant,
It has come to the attention of the Division of Reclamation, Mining, and Safety that a problem noted in a
past mine site inspection report either has not been corrected, or has been corrected, but documentation
of the corrective action taken was not submitted to our office.
Please see the enclosed inspection report for your mine site, issued as a result of past inspection of this
site. The problem noted in this report, that our records indicate remains unresolved, has been
highlighted.
To avoid potential future enforcement actions for non-compliance with State Statutes, Rules and
Regulations, and/or conditions specified in your permit for this site, documentation of compliance with the
corrective action specified for the indicated problem must be submitted to our office by February 27,
2007.
