ORIGINAL-PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_~ga~_ns;e <br />INSPECTION DATE: 4/R/OS INSPECTOROS INITIALS: ~- <br />OBSERVATION <br />The Division conducted a possible problem compliance inspection of the site <br />on 4/8/05. Present during the inspection were Mr. John Miner and Mr. Judd <br />Allen the operators. When the site was inspected on 11-17-04 and 11/24/04, <br />the Division had identified possible problems to the General Mine Plan, and <br />had requested the operator to address the issues noted by January 27, 2005. <br />The operator requested and was granted two extensions to be in compliance <br />with the identified problem by the end of March 2005. <br />The Division had requested the operator to provide the total volumes of <br />inert material that had been placed on site that had not been graded or <br />used to backfill the pits. Proof from the county that they have the <br />required permit to accept such fills on site. (Operator no longer accepts <br />fill materials from construction sites, according to John Miner). Provide a <br />new mining and reclamation map, showing the locations of all major and <br />minor affected areas. The map was also to include areas that are in <br />different stages of reclamation with the locations of topsoil. In addition <br />the Division had requested the operator to remove from site non-mime waste <br />that was noted scattered through out the permit area. Since the present <br />operators are planning to succeed the operation from Douglas County Sand <br />and Gravel, they were informed in order to lower the reclamation liability; <br />operator should start backfilling and grading of pits that have already <br />been mined out. <br />As requested the operator had graded and top soiled most of the East facing <br />high walls to approximate 3:1 slopes. The North side of the active pit area <br />has also been graded to almost 5:1 slope and also seeded according to John <br />Miner. Operator had also placed on the new map the location of the topsoil <br />piles and had removed most of the non-mime waste that was noted scattered <br />through out the permit area. Operator also plans to backfill the North side <br />pit prior to submitting the succession of permit application at the end of <br />summer. Once the SO is submitted the financial warranty will be calculated <br />based on the amount of disturbance noted at that time.
