{Page 2~
MINE ID # OR PROSPECTING ID #: nn-t 9~3-00~~~
INSPECTION DATE: 5-~5-ns INSPECTOR=S INITIALS: inn
This was apre-operation inspection only for the purpose of conducting a field review of the area Continental Materials Corporation
proposes to add to [he permit by Amendment No. 2. Mazk Heffner, of Southwestern Ecological Services, met me at the mine and
accompanied me during the inspection. There was not enough time available to inspect the entire permit area.
The last complete inspection of the perrnit azea by the Division was in October of 2000. Problems identified during that inspection
were subsequently abated. The permit area is due for a complete inspection, which should be completed some time during the present
program year. The bond was reviewed at the time of the last inspection and updated as to the required amount.
The entire 62 acre area planned for addition was observed from high ground on the west side of the proposed amendment area. The
site is a portion of what was formerly Little Johnson Reservoir. The dam appears to have been breeched and the reservoir area has not
been used for water storage for many years. The entire site has established to a good stand of grasses with a few aeeas of shrubs and
trees. There are some wetland areas on and adjacent to the site. The amendment's mining plan avoids disturbing the wetland areas.
The proposed amendment area adjoins the east side of the present Phase 4e area and will be mined as an extension of the phase. It is
apparent from field observation that the mining and reclamation plans proposed in the amendment have been carefully prepared to
account for and prevent any potential negative impacts to structures, groundwater and wetland areas on and near the site.
The inspector did not identify, by this field observation, any site issues that were not already addressed in the amendment application
I & E Contact Address
NAME: Pete Melcher
OPERATOR: Transit Mix Concrete
STREET: P_ (~ Rnx 1030
CITY/STATE/ZIP: Cnlnrnrin Sl1rin~s, Cr7 R09n1-103n
Cc: Harry Posey- DMG via e-mail
Mark Heffner- S.
