(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~i_9z~-53d <br />INSPECTION DATE: -7-~S-n'~ INSPECTOR=S INITIALS: _GRS_ <br />OBSERVATIONS <br />This was apre-operational inspection for a Permit Amendment Application. Mountain Aggregates, Inc. has <br />applied for an amendment to their existing 1 12 (c) permit located approximately one (1) mile west of the <br />town of Empire, Co. The purpose of the amendment is to provide additional information concerning the final <br />design of the reservoir which is currently being constructed. The applicant is also requesting the addition of <br />24.87 acres to the permit area to incorporate some features of the water storage reservoir complex. <br />Prior to conducting the actual field inspection the inspector met inthe on-site field office to review the amendment <br />application, including the proposed locations of the slurry wall, dam and inlet and outlet structures. The revised <br />reclamation plan was also discussed and the location of the acreage to be added was pointed out on the <br />application maps. Persons present during the discussion included Pat Stuart- Mountain Aggregates site <br />superintendent, Phil Buckland- an adjacent property owner, Glenn Porzak- City of Golden water attorney, Maureen <br />Jacoby- Banks & Gesso, LeRoy Marquez- Mountain Aggregates and Steve Jamieson- W.W. Wheeler and Associates. <br />At the time of the inspection mining was taking place. Material was being removed from the pit west of the <br />area where the dam is being built and moved to a new processing and stockpile area east of the dam <br />construction area. Explosive bunkers for the Highway Department have also been moved to the east area of <br />the permit, but these will be permanently removed next spring. The current permit boundaries have been <br />staked and final reservoir bank slopes are being graded. Dam and outlet structure construction is also taking <br />place within the current permit area. Portions of the western part of the pit have been graded io 3H:1 V and <br />seeded as much as eight years ago, these areas are well vegetated. <br />The remainder of the inspection consisted of examining the areas to be added to the permit area. These <br />locations were easily identified, and no major problems were identified. <br />No major issues were identified as a result of this inspection, it is anticipated that addition comments will be <br />made as a result of the Division's adequacy review of the permit application. <br />Cc: Maureen Jacoby -Banks & Gesso, Via e-mail <br />Steve Jamieson -Wheeler & Associates <br />Phil Buckland -Adj.
