(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2003-073 <br />INSPECTION DATE 09/18/2006 INSPECTOR'S INITIALS DEG <br />OBSERVATIONS <br />This routine monitoring inspection of Permit Number M-2003-073 (Juniper Mountain Gravel) was conducted by Deb E. Gbnima <br />of the DRMS. Lafarge representative Julie Mikulas was present during this inspection, but representatives from Moffat <br />Limestone Co., Inc. were unavailable. The pit is located approximately 4.5 miles southwest of Maybell, CO and currently <br />consists of approximately 60 acres where approximately 15 acres have been disturbed. <br />The sign posted at the entrance to the mine site belongs to Lafarge and was well-placed and legible. However, this entrance is <br />shared by Moffat Limestone Co., Inc. as well as Lafarge. Therefore, a sign designating Moffat Limestone Cc., Inc. as actively <br />mining this site is also necessary per Rule 3.1.12(2) (see Problem No. 1 and the corresponding corrective action noted on page <br />3 of this report). The permit boundaries are well-marked and all activities and completed work at the time of inspection <br />appeared to follow the guidelines and regulations set forth by the state and the mining and reclamation plans forthe pit, and no <br />additional problems are noted for this site. The site is stable and no disturbance was observed outside of the approved permit <br />boundaries. <br />There were several large material stockpiles of processed gravels west of the fenceline that runs along the center of the <br />permitted site. The majority of the site is still active and flatly graded, although there was no equipment on site. There appears <br />to be no offsite erosion, and no long-term (greater than 72 hours) water retention was observed. <br />As a final note for future reference, this site is correctly positioned in the DMG online GIS. <br />Responses to this inspection report should be directed to Deb E.
