(Page 2) • <br />MINE ID # OR PROSPECTING ID # M-99-064 <br />INSPECTION DATE OS/11/00 <br />INSPECTOR'S INITIP.LS ESC <br />This was an inspection of the Lyon King Quarry conducted by Erica Crosby of the Division of <br />Minerals and Geology. Luis Vasquez, the operator and permittee was present during the <br />inspection. Weather conditions on site were clear and ground conditions were dry. <br />The 110 Mining and Reclamation Permit was issued to Luis M. Vasquez on August 23, 1999 to <br />mine flagstone on a permit area of 9.9 acres. Quarrying was taking place on less than one <br />acre within the permit area. The area was specifically inspected because it is located <br />directly west of Permits M-87-141, M-86-115 and M-87-108 that are operated by Blue Mountain <br />Stone (" BMS" ) No permit overlapping was noted with the BMS quarries. <br />Permit boundary markers were posted to delineate the permit area. The markers appear to be <br />consistent with the approved mine plan map. The Division was unable to locate a mine <br />identification sign on site.
