MINE ID # OR PROSPECTING ID #: nn_~ ggg_n~i <br />INSPECTION DATE: ~-~ R-nd INSPECTORd INITIALS: inn <br />OBSERVATIONS <br />This was a monfforina inspection mode at the request of Jerrv Novak of Puloff Rock Investments. LLC. Mr. Novak <br />requested the inspection because the property has been sold to Pikes Peak Community foundation, minina has <br />Middle Pit M-1999-072. These three adiacent 110c permits were approved prior to the Board's present policy to <br />not allow adiacent 710c permits. The three permit areas total approximately 30 acres. Mr. Novak and Mr. <br />Hagan were present for the Inspection. A separate Inspection report has been Generated for each permit, <br />However, each report reads the same. <br />Most of the surface of the combined permit areas had been disturbed by borrow/minina activities prior to the <br />ffner material either overburden or waste material hauled in from other locations. Nearly the entire site is <br />disturbed by scattered small pits, roads. piled materials and other disturbances. The area of the permits is at the <br />top of a hill/ridge. There is almost no watershed to contribute runoff to the affected area. <br />reclamation plans for all three permits requires that the entire disturbed area be graded to a relatively level <br />The permit requires use of a grass seed drill to plant the approved seed mix in a prepared seedbed fertilized <br />with 40 pounds each of available nitrogen and phosphorous. The planted area is to be mulched with 4.000 <br />pounds per acre of straw crimped into the surface The seed mix is 2 1 Ibs /ac of Luna Pubescent Wheatarass <br />9 1 Ibs./ac. of Manchar Smooth Brome and 2.4 Ibs./ac. of Western Wheatarass. The seedinG rates are on a pure <br />live seed fPLSI. <br />No problems were noted. <br />the Inspector observed very little of such material and suggested that any old piles of material that are <br />supoortina plant Growth be used as topsoil. <br />Revision. with the inclusion of such a letter. <br />
