~ • • (Page 2) • <br />AMINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 9-24-99 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS BK <br />The Division conducted a joint monitoring inspection of the site with representatives from <br />CDOH&E. The main purpose of the inspection was to familiarize representatives from CDOH&E <br />ABOUT THE OPERATION. Present during the site tour were Miss Carla Lenkey and Mr James <br />Dominguez of CDOH&E, Dr. Harry Posey of DMG, Mr. Scott Lewis and Mr. Mason Estes of CC&V. <br />While in the office, Mr. Lewis gave a brief over view of the entire operation. The physical <br />inspection began north of the main Cresson pit area where the operator has placed a temporary <br />viewing area using a haul truck bed. Mr. Lewis gave a brief description of the operation by <br />pointing out the areas he covered during the office briefing. Inspected the main pit area <br />where active mining was taking place. Inspected the Ironclad and Globe Hill areas. a brief <br />description of the area and its history was presented by the opeartor. Inspected the crusher <br />area. a brief tour of the main crusher area was conducted.Inspected the Phase II expansion <br />area, drain cover fill was being placed in the lined area. Golder QA/QC PERSONNEL WERE ON <br />SITE OVER SEEING THE PLACEMENT AS REQUIRED IN THE AMENDMENT APPROVAL.While taking a reading <br />from the Low, and High Volume Collection systems CDOH&E representatives went to the under <br />drain discharge area and AG 1.5. AG 1.5 IS AN NPDES DISCHARGE POINT. THE FOLLOWING READINGS <br />WERE TAKEN FROM THE HIGH VOLUME AND LOW VOLUME COLLECTION SYSTEM PUMPS. LOW VOLUME PUMP #1 <br />MEASURED .43 FEET OF HEAD, PUMP #2 MEASURED .26 FEET OF HEAD. HIGH VOLUME PUMP # 4 READ <br />31.7, PUMP #5 READ 30.7 AND PUMP #6 READ 37.9.
