(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~ 9zx-020- <br />INSPECTION DATE: - 04J~~rn6 INSPECTOR'S INITIALS: _ KAP <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Minerals and Geology's routine monitoring <br />program of in-stream operations, and also served as training for newly hired EPS's with the Division, <br />regarding in-stream gravel operations. Present at the inspection were Kate Pickford, Wally Erickson, <br />Chris Rowe, Deb Gonima, and Rick Wenzel, DMG, and Marcia Talvitie, Four Corners Materials, Inc. <br />Weather at the time of the inspection was clear, calm, and approximately 55 degrees. <br />2. The site has been altered since the time of the previous inspection. Along the north end of the site, <br />where previously there was a historic pit that had captured the Animas River, a road has been installed <br />on backfill that was used to isolate the pit from the river. The pit is now a lake, located west of the river <br />channel. Some of the material used to isolate the pit extends well into the current river channel, and it is <br />unknown whether this configuration is in compliance with Army Corps of Engineers guidelines. <br />3. Grading has occurred on the south end of the site as well. The area where the jetty was installed and <br />temporary blocks were used to build up the height of the jetty to prevent 2005 runoff from cutting across <br />the top of the jetty into the pond, has been backfilled, thus raising the level of the area behind the jetty <br />fo the level of the fop of the temporary blocks. As was stated in a reported related to an inspection of <br />the site conducted on June 5, 2005, if the operator intends to have the temporary blocks remain after <br />reclamation, a technical revision to the permit will be required. The technical revision must include <br />plans that certify that the final jetty structure will remain stable in a 100 year, 24 hour storm event. <br />4. An outflow channel has been installed between the pond resulting from onshore mining and the Animas <br />River. The channel is lined with rip rap and appears to be well constructed. <br />5. Activity at the site is in compliance with the permit and with the Construction Materials Rules and <br />Regulations. The current bond amount of $25,1 13.00 was confirmed as adequate for reclamation of the <br />site during the inspection, conducted on June 5, 2005, and was therefore not recalculated at this time. <br />I & E Contact Address <br />NAME: Marcia Talvitie <br />OPERATOR: Four Corners Materials.
