<br />• (Page 2) <br />_~ <br />MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 9-27-99 <br /> <br />INSPECTOR'S INITIALS BK <br />The Division conducted a monitoring inspection of the Phase II and Phase III Leach pad <br />expansion areas under construction on 9-27-99. Present during the inspection was Mr. Phil <br />Barnes of CC&V.Drain cover was being placed along the North facing slopes of the Phase II <br />leach pad expansion area. Two Golder QA/QC personnel were over seeing the operation. Phase <br />III backfill, grading and remediation of under ground workings was continuing. Inspected the <br />secondary under drain construction along the west facing slopes of the Phase III leach pad <br />expansion area. An excavator was being used o get it to final grade prior to placing rip rap. <br />Golder Qa/QC personnel Mr. Dednato briefly explained the process and how it was going to be <br />tied in to the main under drain for the area. Inspected the ARD area. The expansion west of <br />the process building for the two auxiliary tanks was taking shape. The foundation for the <br />tanks placement area is was in place. No major problem noted. <br />The following readings were taken from the Phase II HIGH VOLUME AND LOW VOLUME COLLECTION <br />PUMPS.
