<br />(Page 21 <br /> <br />MINE ID # OR PROSPECTING ID # M-82-011 <br />INSPECTION DATE 8-20-96 INSPECTOR'S INITIALS SSS <br />OBSERVATIONS <br />This was a partial inspection of the Hillyer Pit in the town of Silverton in San Juan County conducted to determine how much work <br />would be required to reclaim the pit in it's current condition. This inspection was initiated by requests from the operator for <br />information regarding remaining reclamation to be conducted at the site. <br />The site will need some regrading of gravel piles that are in the river channel and steep riverbanks along the channel in some <br />locations. Several piles of material have been dumped along the riverbank, presumeably as a source of soil material. However, some <br />of the piles contain chunks of old asphalt and other unacceptable materials. These piles will need to be cleaned of unacceptable <br />materials or removed from site. Additionally, there is a pile of broken concrete at the north edge of the pit. The lack of permit <br />boundary markers makes it unclear whether or not this pile is located on or off the permit area. If this material is on the permit area, <br />or if it was generated by the operation, it will need to be disposed of in a proper manner. If this material is not associated with this <br />operation, and is not located on the permit area, the operator must clarify this fact, in writing, to the Division. <br />If the operation does not have Army Corps of Engineers approval to conduct operations in the river, this approval may need to be <br />obtained before the regrading of the river banks and gravel piles, located in the river, can be conducted. <br />Several piles of crushed gravel remain on site. <br />Please be aware, Rule 3.1.12 of the Mineral Rules and Regulations requires that: <br />111 The operator shall post a sign at the entrance of the mine site which shall be clearly visible from the <br />access road and specify the following: <br />lal The name of the operator, <br />Ib) An indication that a reclamation permit for the <br />operation has been issued by the Colorado Mined <br />Land Reclamation Board, and <br />(cl The Permit Number. <br />121 The boundaries of the affected area will be marked by monuments or other markers that are clearly <br />visible and adequate to delineate such boundaries. <br />The sign and markers were not observed during the inspection and should be installed as indicated above. <br />I & E Contact Address <br />NAME JoAnn Vota <br />OPERATOR Hillco Rock, Inc. <br />STREET P.O.
