(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-1999.19n <br />INSPECTION DATE: 11/~~/nd INSPECTORS INITIALS: KAP <br />OBSERVATIONS <br />This inspection was conducted as apre-operational inspection for an amendment to an existing 1 12c <br />permit, adding 212 acres to the permit area. Present at the inspection were Kate Pickford, DMG, Lynn <br />Myer, L.G. Everist, Inc., Maureen Jackovy, Banks and Gesso, and Sharlene Haeger, Colorado Division of <br />Wildlife. Weather at the time of the inspection was clear, calm, and approximately 55 degrees. <br />2. The proposed added area is currently agricultural, and contains some residences. The area runs along <br />a portion of the South Platte River where the operator proposes to maintain a 200-foot setback. None <br />of the area proposed to be added in the amendment has been disturbed by mining activities. <br />3. The amendment proposes to dry mine five new areas in the permit area. These areas will either be <br />surrounded by a slurry wall prior to mining or will be lined with a clay liner at the completion of mining. <br />The operator intends to keep these options available for a later decision for each pit. A slurry wall that <br />was ineffective until May of 2004, when it was reinstalled, has surrounded the current pit. Therefore, dry <br />mining was occurring for nearly three years without a slurry wall and with no evident impacts on <br />neighboring wells. The Operator will be required to supply a survey of neighboring wells and a <br />groundwater model depicting the effects of both scenarios for each proposed pit during the review <br />process for the amendment <br />4. The Division of Wildlife attended the inspection due to concerns related to the winter roosting area for <br />bald eagles along the South Platte River.
