(Page 2)
MINE ID # OR PROSPECTING ID #: ti+-19Sn-24d
INSPECTION DATE:Z n~<a~ INSPECTOROS INITIALS: ~_
OBSERVATIONS
The Division conducted a monitoring and construction progress inspection of the site on 7.03-03. Present during
the inspection were, Messer Tim comer, Mark Tidquits and Terry Mnadziac of CCBV. Prior to the site inspection
an informal discussions were held with Mike Irish of CCBV, about CCBV 'S PLAN TO APPLY AMMONIA DIRECTLY
TO THE ORE BODY TO RAISE THE PH TO BETWEEN 10.5 TO 11.0. He explained why CCBV chose the Ammonia in
addition to the CAO that is still being utilized during the Gold recovery process. The Division listed the possible
concerns. Since it was not a formal meeting, the Division agreed to discuss the matter with Mr. Phil Barnes,
Manager Environmental Affairs, on how to best address the Division's possible concerns.
The Physical inspection began at the Phase IV leach pad PSSA area. Temporary power to pump the solution
back and forth the ARD building was in place. Transducers to measure both the upper and lower pond
hydrostatic head were in place. According to Mark, the transducers were properly calibrated and appeared to
be functioning as designed. Only drain cover deployment was taking place at the time of the inspection. The
first lift ore has been deployed for the most part in the PSSA AREA. CCBV plans to apply solution as soon as
possible.
Inspected the Phase I AND II pregnant solution collection areas and recorded the following readings from the
Phase I AND II HIGH AND LOW VOLUME SOLUTION COLLECTION TRANSDUCERS.
Phase I High Volume solution collection transducer # I was recorded at 35.5 #2 was at 38.1 and # 3 was at 38.6
Low volume solution collection fransducer # 1 was at .21 and # 2 was aF .53. Piezometer for the area was
recorded at 41.3 feet.
Phase II High volume solution collection transducer #4 was at 26.0 #5 was at 20.8 and # 6 was at 26.1
Low volume solution collection fransducer #1 was at .13 and # 2 was at .54. Piezometer for the area
was recorded at 28.7 feet. (ALL TRANSDUCER READINGS ARE IN PEST OF HYDROSTATIC HEAD)
Inspected the external pond and collected the following readings from the low volume solution collection
transducers. # 1 was recorded at 12.8 and # 2 was at 9.6 (all readings are in inches of hydrostatic head)
Inspected the under drain area both the South under drain and the 41nch pipes were discharging, the
discharge was being pumped back onto the leach pad. No major problem noted. Both lower ponds were
holding some water from the recent rainfall, but were not discharging.
f R E Contact Address
NAME: MR.
