(Ppge 2~ <br />MINE ID # OR PROSPECTING ID #: +~~-tv~~-~e~ <br />INSPECTION DATE: OSLfL'T/nz INSPECTOR'S INITIALS: -KAY <br />OBSERVATIONS <br />l .This inspection was conducted as part of Division of Minerals and Geology's four-year monitoring plan. <br />Present at the inspection were Kate Pickford, DMG, Don Coram, Gold Eagle Mining, Inc., and Dan Burns, U.S. <br />Department of Energy (DOE). Weather at the time of the inspection was clear to partly cloudy, calm, and <br />approximately 80 degrees. <br />2, An appropriate sign is posted on the ore bin at the site. <br />3. The site contains waste rock piles, an ore bin, a portal, two shafts, a vent hole, and an access road. A <br />previous inspection was conducted at the site on October 18, 2002. At the time of the previous inspection, it <br />was noted that the portal had been partially backfilled. The backfill material has since been removed from the <br />portal entry and a locked, metal gate restricts portal access. Some water was ponded outside the portal, but <br />Mr. Coram indicated this was a result of precipitation. Storm water management is evident at the site in the <br />form of sediment control structures and culverts to transport runoff within the site. <br />4. Mr. Coram stated that Gold Eagle Mining, Inc. will be reopening the mine soon. Bond for this site is held both <br />by the Division and the Department of Energy. The DOE currently holds $23,250.00 and the Division holds <br />$100.00.
