INSPECTION REPORT <br />1 <br />June 23, 24, & 26, 1992 <br />Deserado mine <br />Western-Fuels, Inc <br />PO Box 1589 <br />Rangely, Colo <br />Personnel Present During the inspection: <br />Tim Chervick Western-Fuels <br />Wayne Heilmann Western Fuels <br />Jeff Dubbert Western Fuels <br />Larry Routten Colorado Mined Land Reclamation <br />Janet Binns Colorado Mined Land Reclamation <br />Gary Fritz Office of Surface Mining\Albuquerque Field Office A244 <br />Weather and Ground Conditions During the Inspection: <br />Warm, partly cloudy and dry with no precipitation 24 hours prior to the <br />inspection <br />GENER.IL COMMENTS <br />This was a complete oversight inspection. State officials were notified <br />regarding my schedule and were able to meet me at the mine for a joint <br />inspection. As noted in the heading of the narrative for the report, there <br />is a break in the time, when I left the mine, initially i indicated in the <br />first closeout that the inspection was completed but I decided to return to <br />the mine to gather more information on the waste disposal areas and mine <br />water discharge. This was prompted following my phone call with my <br />supervisor on the day following the first portion of the inspection. I <br />tried to reach him via a phone call as we closed out on the previous day <br />but he was not available. I called the next day as I started another <br />inspection on another mine and discussed my concerns with him at that time. <br />He suggested that I return to the mine.. I told him at that time that <br />there could be a problem with scheduling since I was on the other <br />inspection with no idea as to how long it would take as well as being <br />several hundred miles away from the mine. <br />i was able to complete the inspection of the other mine and return to this <br />area for additional review. I notified Mr. Routten of my intentions to <br />return to the mine on thursday evening, the 25th. He said that he would <br />consider returning to the mine but was not sure that he would. He did not. <br />I do not consider his absence as being a problem in this matter nor have I <br />indicated that the State inspection was not complete on the MEIR form <br />included as part of this report. Mr. Routten and Mss Binns are aware of <br />my concerns in this matter Irk but felt that they did not find sufficient <br />reason to pursue it more during their inspection. <br />I returned to the mine to document in more detail the <br />waste disposal areas. <br />f~ e - - °w - This <br />detail with the same company personnel during <br />the mine in November of 1990.
