(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-1985-n~v <br />INSPECTION DATE: 6J~nx INSPECTOR S INITIALS: ~Al' <br />OBSERVATIONS <br />This inspection was performed as part of the four-year monitoring program conducted by the Division of <br />Minerals and Geology. Present at the inspection was Kate Pickford, DMG. Weather at the time of the <br />inspection was clear, calm, and approximately 70 degrees. <br />2. Access to the site is via a dirt road off the south side of Park County Road 12, west of Park City, north of <br />Fairplay off Hwy 9. There is no sign posted at the mine entrance (see problem 1, page 4). Disturbance <br />at the site consists of activity on the west side of the site where it appears the latest mining has <br />occurred, and three ponds used for sediment or tailings. There is a network of culverts on the site <br />3. There is an excessive amount of materials, equipment and apparently inoperative equipment at the <br />site, as well as multiple tanks and drums that pose potential problems at the site (see problems 2 & 3, <br />page 4) The following is an inventory of the materials stored at the site: <br />On the access road <br />2 -dozers <br />2 - 55 gallon drums near one of the ponds that are leaking <br />• 3 - 5 gallon buckets, full <br />• 1 -old pickup truck <br />10 - 12" 16 foot steel pipes <br />1 -semi trailer <br />1 -shipping crate <br />• 1 - 15,000 (approx.) gallon storage tank <br />1-conveyor <br />• 1 - 55-gallon drum, leaking, near large storage lank, unknown contents <br />• 4 - 55-gallon drums, near storage tank, unknown contents <br />• 7 - 12", 20' steel pipes <br />2 -mobile homes <br />• 1 -refrigeration trailer <br />• 3-junk pickup trucks <br />4 - 55-gallon drums, near trailers, unknown contents <br />• PVC pipe <br />• Misc-scrap metal; motors, trailers, platforms <br />Near the active workings of the mine <br />3 - 12", 20' steel pipes <br />• 1 -office trailer <br />1-hopper <br />• 1 -fuel tank <br />1 -utility trailer <br />• 1 -haul trailer <br />1 -generator <br />• 2-55-gallon drums, leaking, unknown contents <br />Near clean water pond <br />• 1 -pump <br />1 -fuel tank <br />• 3 - 5-gallon buckets, one spilled, unknown contents <br />A bond calculation will be conducted as a part of this inspection. Costs for removal of the above <br />materials will be included in the calculation unless the operator removes them prior to the follow up <br />inspection for the site, noted in the problems section of this report. <br />
