(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1980-246 <br />INSPECTION DATE 8/19/04 <br />INSPECTOR'S INITIALS RCO.HHP <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The household and/or agricultural trash that was dumped onsite is not in compliance 10/30/04 <br />with the permit's approved plan. PB or PV: PB <br />TOPIC(S): MP <br />CORRECTIVE ACTION: The operator must remove the trash and dispose of it in a _, _, <br />proper facility. Evidence of completing this action must be received at this office by the <br />date shown at right. <br />2. The stockpiled topsoil is being allowed to degrade through erosion and sloughing off 10/30/04 <br />highwalls. PB or PV: PB <br />TOPIC(S): TS <br />CORRECTIVE ACTION: The operator must move and/or reconfigure the stockpiled _, _, <br />topsoil to a more stable location, and seed the piles. Evidence of completing this action <br />must be received at this office by the date shown at right. <br />3. The site is becoming infested with knapweed, astate-listed noxious weed, and shows 10/30/04 <br />no evidence of being controlled. PB or PV: PB <br />TOPIC(S): RV <br />CORRECTIVE ACTION: The operator must submit a revised weed control plan, as well _, _, <br />as the commitment to implement it. (A copy of this report is being sent to the county <br />weed control manager, who may contacted for assistance with the new plan. See <br />address on page 3.) The weeds should be treated soon, but the required written plan <br />and commitment must be received by the date shown at right. <br />4. There is spilled waste oil and improperly-stored fuel onsite, which are present and/or 9/30/04 <br />future site contaminants. PB or PV: PB <br />TOPIC(S): AT <br />CORRECTIVE ACTION: The operator must remove all waste oil and dispose of it _, _, <br />properly. The fue! tank must be changed so it provides impermeable secondary <br />containment. Evidence of completing these actions must be received by this office by <br />the date shown at right. <br />5.
