MINE ID # OR PROSPECTING ID # M-2001-063
INSPECTION DATE 10/2/01
As noted above, natural drainages on the site will have to be crossed by vehicles and equipment, and description of all
structural work performed to ensure stable crossings must be provided to this office. One area in particular, being the lower
50 or 100 feet of the old road leading up to the loffsitel Unity Mutual Mine portal was indicated by the operator as a
possible vehicle parking or equipment storage area. This use would require another one or two culverted stream crossings.
Again, if culverts are needed, please do not use galvanized steel or steel. The operator also runs the risk of contaminants
from the equipment directly affecting the stream. The portal area would be preferable as such a parking or storage area.
The new maps are a good step in the right direction, but require additional information to be added to them. Please refer
to Rule 6.2.112) for map standards and Rule 6.3.5(2) and 131 for required content of the mining map and reclamation map
(such as labeling the permit boundary line, ore storage areas, soil stockpiling areas, potential dump area, stream crossing
structures, water sampling locations, equipment storage and vehicle parking areas, landowners, etc.). The revised maps
may be submitted along with the other adequacy responses to be submitted or separately.
There were no further observations or discussion during the inspection. This inspector appreciates the operator's effort
and expense involved with being present for this inspection. All responses or questions about this inspection report should
be directed to this inspector at the Division's Durango Field Office. The address is: Division of Minerals and Geology, 701
Camino del Rio, Room 125, Durango, Colorado 81301; telephone 970/247-5193, or fax 970/247-5104.
(Page 3) •
INSPECTOR'S INITIALS RCO
I & E Contact Address cc: Tom Gillis, DMG
NAME
OPERATOR
STREET
CITY/STATE/ZIP
Wavne Giesv
Midas Minino Co.
