(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1994-097 <br />INSPECTION DATE 1-29-2003 <br />INSPECTOR'S INITIALS SSS <br />This was apre-operational inspection of the new access road area proposed by United Companies, through application for <br />Amendment No. 2 (AM-02) to Permit M-1994-087. <br />Several structures were observed that are not illustrated on the AM-02 maps and are not addressed adequately in Exhibit S. <br />Additionally, the owners of several of these structures were not sent copies of the public notice as required. The operator will <br />need to carefully inspect all areas within 200 feet of the proposed addition to the permit area and ensure all structures are <br />adequately addressed both in text and on the maps. Also, documentation must be submitted to prove that all owners of record <br />of surface lands (including structures) on or within 200 feet of the proposed addition to the permit area have received the <br />required public notice. <br />Some of the structures noted during this inspection include: Near the junction with LaSalle Road; a buried gas line, a buried <br />fiber optic communication line and connection box; an overhead powerline with several power poles, a culvert under LaSalle <br />Road, a pig wire and 3 strand barbed wire fence along the south side of LaSalle Road, a similar fence perpendicular to LaSalle <br />Road running to the south, and a second freeze proof hydrant. <br />The owners of record described above who have not yet received adequate notice must be provided a 20 day comment period <br />following their receipt of notice, or they must waive this right in writing.
