III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations <br />made duririg the inspection. Comments also describe any enforcement actions taken during <br />the inspection and the facts or evidence supporting the enforcement action. <br />006-NE4 Gulch Channel <br />October 2005 inspection identified concerns with channel shape, depth, and presence of ruts, and <br />maintenance was recommended. Please refer to DMG inspection report issued October 12, 2005 <br />for detailed observations. <br />From general observations made during the June 2006 inspection, it appeared that maintenance had <br />been performed on segments of concern, however detailed re-inspection was not conducted, due to <br />time constraints. Operator should conduct a check of the repaired channel to ensure that <br />concerns addressed in the October 2005 inspection were addressed, and to verify that the <br />channel complies with the approved design. <br />006-NE1 Gulch Channel <br />October 2005 inspection identified concerns with channel shape, riprap placement, presence of rills, etc <br />Maintenance was recommended for various segments. Please refer to DMG inspection report <br />issued October 12, 2005. <br />From general observation it appeared that maintenance had been performed on segments of concern, <br />however detailed re-inspection was not conducted, due to time constraints. Operator should conduct a <br />check of the repaired channel to ensure that concerns addressed in the October 2005 inspection <br />were addressed, and to verify that the channel complies with the approved design. A major <br />segment of the 006-NE1 channel is shown in the left center of Digital Image 0164. <br />006-E1 Channel <br />Upper Section (HH to SS on Exhibit 20-2) <br />Channel profile on permit Drawing 20-5.1 depicts channel slope increasing from approximately <br />13% at HH (upper end above Ridgeline Road), to approximately 21% along the steepest <br />segment, which ends approximately 500' upstream from SS. From the lower end of the steep <br />segment, design profile depicts a 12% gradient segment extending down to SS (the 006-E2 <br />confluence}. The permit channel design sheet (Figure 20-1.1) specifies a trapezoid channel <br />with bottom width of 2', top width approximately 6', depth 8.4", and riprap with 6" D50 for <br />slopes between 8% and 27%. <br />From our inspection, upper segment above road was somewhat steeper than indicated <br />on design profile, with steepest sections ranging from 15% to 19%. The channel above <br />the road was a "V" shaped channel and was not riprapped. The lower segment of the <br />channel just upgradient of the road was Tilled 6"-9" deep. There was also shallow <br />Tilling parallel to the outside of the channel due to flow concentration along a berm that <br />had resulted from channel construction. For the section of HH-SS upgradient from <br />the Ridgeline Road crossing, channel shape and liner do not appear to be in <br />conformance with approved design for the gradient. Modification is necessary to <br />conform with approved design. <br />• Downstream from the Ridgeline Road crossing, the segment is a riprapped trapezoid <br />4 <br />
